CORI forms are now available online for:
- Massachusetts families:Please print and fill out a CORI form and bring it to the school office with official ID at the start of school.
- New Hampshire families: Please print, fill out, and have notarized this form ,
and send to the NH State Police. Include this form for a fee reduction.
Completed CORI forms must
be in by the end of September, since field trips and events happen immediately
when school opens in the fall. These forms are only valid for parent/family volunteers. Parents must come in to school with their license or any
other government-issued picture ID, or a copy, for verification by Mrs. Ridgley, who can only CORI Massachusetts residents
School volunteers must also take the
"Protecting God's Children" training offered by the Archdiocese.

Donnelly's to sew onto jumpers and vests. Polo shirts must have an embroidered logo if they are worn without a logo'd vest or
sweater over them.

Athletic Participation Information
Each student athlete and his/her parent are required
to read the Athletic Handbook. There is an "Acknowledgement Form" that
be signed stating that you have read the Handbook prior to participation in any of the sports programs.
The Athletic Packet
provides permission forms, registration
forms, uniform order forms, and other important information for all
sports this year. Please print, complete,
these forms by the stated deadlines.
